Welcome To Major Changes to Clinical Health Clearance Process - begins SP06

IMPORTANT INFORMATION REGARDING CLINICAL CLEARANCE AND CLINICAL PLACEMENTS

We are delighted to let you know that we have collaborated with the University Health Service to make the clinical clearance process easier for all of us. The following changes have been made:

• New students will be required to submit a photocopy of the University Health Services Immunization History Form to the College by the submission deadline.

• Continuing students will still need to obtain a PPD screen annually, CPR certification every two years and provide verification of health insurance annually. 

• The College will now accept a photocopy of your insurance card as proof of insurance.  If you have the University’s insurance coverage, there is a form that you can complete at the Reception Desk at the College.

The clinical clearance deadline for the Spring 2006 semester is Friday, January 6, 2006 at 5:00 pm.  We will only accept photocopies of the required documentation.

Please note that any falsification of any forms will be dealt with in accordance with University and College policy.

Spring 2006 clinical placements will be sent out via UMass Boston email within the next two weeks.  If you would like to request a change in your clinical placement, you must do the following:

• Find a classmate that is willing to switch with you
• Both parties must come to the College to fill out a Request to Switch Clinical Placement Form
• We will begin to review the requests on December 14th and you will be notified via your UMass Boston email of the decision.
