Dear Nursing Students,

              Due to ongoing updates in agency and faculty placement confirmations, along with unforeseen staffing changes, clinical placement information will not be finalized until early-mid August.  Thank you for your patience.

              Please remember that the deadline for handing in clinical clearance packets is August 11, 2006 at 5 pm.  Documentations must be valid through December 15, 2006.

-Health insurance verification

-TB test or chest Xray documentation

-MMR 2 doses or MMR titer documenting immunity to Measles, Mumps, and Rubella

-Hepatitis B 3 shot series-if not started, must be started immediately

-Tetanus immunization within past 10 years

-Varicella titer documenting immunity to chicken pox

-CPR certification (American Heart Association-Healthcare Provider)

-CORI clearance (please note that some agencies are now also requiring SORI-sex offender clearance and student fingerprinting)

-RN license, if applicable
